Advanced Diabetes Dietitian (LOCALITY)
Band 7, full time permanent position
A brand new and exciting opportunity is available within the speciality of diabetes dietetics in Dorset. The role supports diabetic patients closer to home by being a locality position, working closely with multidisciplinary primary and secondary care diabetic teams.
As part of Dorset’s Accountable Care System journey, six million pounds are being invested in Dorset’s Integrated Community and Primary Care Services. The aim is to develop an integrated workforce within localities, with a strong focus on partnership working. Services will be based upon the needs of people in each locality, and will focus on providing;
- Services for people with high intensity and complex needs,
- A proactive approach to care for people with medium intensity needs caused by long term health problems
This role is a full time permanent position. It is a brand new role with a strong focus on group education and building integrated locality dietetic services for diabetes patients. The post holder will provide a specialist patient-centered dietetic care to individuals and groups of people with diabetes. This will include providing expertise to care homes and house bound patients.
The role also will work closely with the secondary care advanced diabetes dietitian to ensure services are continuous between community and secondary care.
It will require an enthusiastic and independently motivated dietitian to develop and provide patient group education that meets the remaining needs of the locality diabetes population.
The post holder will be based in Dorchester at the Dorset County Hospital NHS Foundation Trust and will cover the Weymouth and Portland locality of Dorset.
You will become part of the Nutrition and Dietetic Team at Dorset County Hospital NHS Foundation Trust. This team consists of acute, community, Home Enteral Nutrition, nutrition nurse and administrative personnel.
The post holder will be working autonomously, but will have close working relationships with the Weymouth and Portland Diabetes MDT, as well as the secondary care advanced diabetes dietitian.
